Just putting this here for others to see what requests have been made already.
I will share the whole requests and responses received so far, when I get time.

Any KPI dashboards, quality-monitoring reports, or oversight documents summarising system-wide maternity outcomes (e.g. caesarean rate, induction, perineal trauma, stillbirth, neonatal admission).

Any governance documents showing how these reports feed into decision-making or service reviews at board level.

Please include electronic copies (PDF, Word, or Excel).

Operative/assisted births

Perineal trauma or episiotomy

Post-partum haemorrhage

Neonatal outcomes (stillbirths or neonatal admissions/transfers)

Any dashboards or performance summaries presented at those meetings containing these indicators.

I would prefer the documents in electronic (PDF or Word) format.

I previously requested AEQUIP documentation but have not yet received it. I therefore restate and expand that request under the Freedom of Information Act 2000.

Please provide for the past five years (2020-2025):

All AEQUIP (A-EQUIP) implementation reports, annual assessments, or PMA supervision summaries relating to maternity staffing and safety culture.

Any risk assessments, internal reports, or governance papers produced in connection with the suspension of home birth services (for example, in 2023 and 2025), including documents showing:

 a. When and why services were suspended or restricted;
 b. What measures were taken to maintain safe staffing and continuity of care;
 c. Any AEQUIP findings or reflections logged as part of that process.

Records of incidents or decisions where women were redirected to other NHS Trusts or neighbouring maternity units due to staff shortages or service suspension.

Copies of internal correspondence or meeting minutes where AEQUIP outcomes or supervisory feedback were discussed in relation to these service changes.

 b. Safe staffing and escalation requirements
 c. Continuity-of-care commitments
 d. Co-production or user-engagement expectations
 e. Monitoring or reporting duties for service changes or suspensions

Any Key Performance Indicators (KPIs) or quality metrics linked to the above.

The total budget allocation for maternity safe staffing (including midwifery and maternity support workers) for each of the past five financial years (2020/21, 2021/22, 2022/23, 2023/24 and 2024/25 to date).

breakdown of how that budget has been divided between:
 a. Midwifery recruitment
 b. Midwifery retention initiatives
 c. Agency/bank staff
 d. Training and supervision
 e. Community/on-call cover, including home birth provision

The amount of unspent or reallocated budget in each of those years.

The amount held for contingency or emergency staffing and how such funds are accessed when community or home birth cover is at risk.

Copies of any risk assessments, escalation plans or decision-making protocols used when maternity staffing falls below safe levels or when home birth cover is withdrawn.
